So - this seems to be tantalisingly close to existing (link).

I'm not the first to note that a round or oval, pink, two-hole button looks a lot like a pig nose.

A shirt for adults, with oval pink two-hole buttons and a pig design aligned with the buttonholes. Either all the way down the front, or just on the pocket.
