Paper-producing ballpoint

This thicker than normal pen would enclose, in its upper half, a 3/4 inch (2 cm) wide Post-it® roll. A small wedge would ease unrolling, and the paper would proceed the length of the pen and exit through a diagonal, serrated slot between the index and middle fingers.

To use, grab the edge of the adhesive paper at the slot, pull out one inch under the pen tip and, holding the edge in place with a finger, proceed to write. The notepaper will feed out as you write and can then be torn off at the pen slot.

Didn't see this at the time, but this stuff is pretty Baked. Don't know about actual paper, but plenty of adhesive writable tapes and films are out there, in pen-like dispensers. Poke around some in your local stationery or artist's supply store.
