This may well have been pre-baked, but most diseases that kill you also make you awfully skinny just prior to the grim-reaper appearing. Some of the older diseases that are now almost extinct because they are fully curable should be used as weight loss agents. ie: Doctor injects you with Diptheria or Bubonic Plague or Typhoid, and as you lose continue about your daily life you very quickly lose weight. Before you die, you're given the life-saving penicillin or whatever. Water-Cooler conversation "Bob, you're looking terrible but losing weight at last. How do you do it?" "Hi Terry, just got a touch of Bubo, the weights just falling off me."

BUT, maybe some of the minor illnesses like Malaria, Tetanus and Mumps wouldn't be too bad.

People who find it hard to lose weight would line up to get sick.

I'm sure someone suggested using sickness as a dieting mechanism before. All I could find was the link below, tho.

Didn't they use tape worm egg tablets for this in the fifties?. When you'd lost enough weight, you took a second tablet which "wormed" you and stopped you losing weight.
