       All I found were cosmetic surgeons trying for some sort of beauty or normality. I understand that shapes could be tattooed, but the idea as conceived would require some sort of skin- or even nipple-graft, and that's a bit weirder than that.

       Women with faint or non-existant nipples is more common than you think. Women who get fat tend to cause their breasts to stretch and subsequently the nipple flattens out.   

       This would be a jaw dropper in and of itself if the woman at the strip club removed her top to reveal two perfectly shaped hearts. It would be more interesting from a scientific perspective than it would be erotic. I had seen some of these breast reduction surgeries on TV where they just toss the nipple around until they dig out all of the meat that they want to remove.. and then the surgeon is like, "lets put the nipple here.. hrmph.. nah.. lets put it here." And during this time of contemplation, the surgeon can cut out the desired shape.   

       As far as the tatooing aspect of it Richard Pryor talked about this in one of his standup routines. He said something to the effect of how the woman had an eye tattooed there and for a moment he thought that the eye winked at him.
