sewing each and every feather to the fabric would be kind of prohibitive, wouldn't it? Besides, you would need several sheets inside, as two layers of feathers would make for a very flat pillow.

Perhaps some sort of sponge, contoured material between the fabric to which the feathers are sewn and the 'outer' layer?
