When you push this public restroom door to open it, the door stops midway, and you hear a dull thud, followed by "Owwwwwwww! Argggggh!". Feeling terribly guilty and positive you have injured someone who was just about to exit, you timidly and sadly peer around the back of the door to find: Nothing.

I suggest maybe a dummy that can collapse after the sound, thereby ensuring immense anxiety for the user. This is evil.
Oh, and what is more: if the person leaves the scene they will have to deal with the guilt of leaving someone harmed and unattended to. This is extra evil. Later, when their friends tell them it was just a dummy, the user will have to bear the admittance of being so cruel as to leave someone harmed and unattended.
